What to Expect

Welcome to Sunmi Naturopathic Health. You have just taken an important step towards optimizing your health.

Getting Started

Before your first visit, please sign up for our online patient portal. (Once your appointment has been created you will receive a patient portal sign up E-mail.) From there you can fill out and submit your initial patient intake and consent forms.

Please give yourself enough time and complete the forms as thoroughly as possible. It usually takes about 30 minutes to complete them. If you have any questions regarding the form, leave the section blank and we will go over it at your appointment.

If you prefer filling out the form manually or have trouble filling out the intake form online, the initial intake package is also available at www.sunmihealth.com “What to Expect” or “Patient Portal” pages. Please complete and bring all forms with you to your first appointment.

First Visit

The first visit includes an in-depth naturopathic assessment of your health concerns, medical history, and health goals. Dr. Cha will review your completed health questionnaire with you, ask detailed questions relevant to your health concerns, run a screening urine test and perform a physical exam.

What to bring with you:

  • New patient intake forms (No need to bring these forms if you already submitted them online.)
  • List of supplements and medications (including brands and dosage)
  • Copy of your most recent blood work (if possible)


Second Visit

During your second visit, Dr. Cha will review your concerns and discuss with you Naturopathic treatment options that are specific to your lifestyle and health needs. Together, you and Dr. Cha  will determine the best way to approach your health concerns, making your treatment plan individualized to you.



Follow-up appointments are essential to monitor progress, to refine and adjust treatment care to continue health progression. The number and frequency of follow up visits is case dependent.


Payment & Reimbursement

Each patient is responsible for payment of naturopathic services at the end of each appointment. Dr. Cha will provide you with a receipt that contains all the necessary information for your health insurance company. For reimbursement, you then submit the provided receipt to your health insurance company.


Health Coverage

Most companies cover Naturopathic Medical care through their extended health care insurance and you may already be paying for this care. Check your benefits package or contact your health insurer for further details.
